• Other Plans

  • For other plans, I am considered an out-of-network provider. If you wish to submit claims for reimbursement of services through an insurance company for which I am an out-of-network provider, you will be provided with procedure and diagnosis codes to assist in the process. My office, however, does not bill insurance companies directly, with the exception of Blue Cross/Blue Shield and AETNA.

Mental Health Coverage

• To determine mental health coverage, you can ask your insurance company the following questions:

  • Do I have benefits for mental health services?

  • Do I have a deductible and if so, has it been met?

  • How many sessions per year does my plan cover?

  • Do I have a co-payment and if so, how much is it?

  • What is the coverage amount for out-of-network providers?

  • Is pre-approval required from my primary care physician?

Fees for Services

• Initial Assessment (60 minutes): $225

• Individual Psychotherapy (50 minutes): $200

• Couples Psychotherapy (50 minutes): $225

• Group Psychotherapy (90 minutes): $75

Payment

• Payment is due at the time of service. I accept personal check, cash, or credit card.

Reduced Rates

• Reduced rates will be considered on an individual basis.

24-hour Cancellation Policy

• Once an appointment is scheduled, you will be expected to pay for it in full, unless you provide 24-hours advanced notice of cancellation.
